Identification de profils de trajectoires typiques dans les études cliniques ou épidémiologiques
Une trajectoire est constituée par les mesures longitudinales d’un biomarqueur chez un individu. L’identification de profils typiques de trajectoire permet de classer les individus, et d’avoir une meilleure connaissance de l’hétérogénéité naturelle entre les patients. Le lien entre les groupes de trajectoire et le pronostique peut ensuite être recherché, ainsi que des facteurs prédisposant à l’appartenance à un groupe donné.
Deux travaux principaux ont été menés dans cette thématique de recherche.
Application à la recherche de facteurs cliniques prédisant la réponse à un traitement antihypertenseur
Recherche des facteurs de réponse au traitement lorsque la réponse est évaluée par des mesures répétées de pression artérielle. Travail réalisé en partenariat avec le Pr Gueyffier de l’équipe EMET.
Adaptation des méthodes à des biomarqueurs binaires ou résultant d’un comptage
Participation aux tâches 3 (KmlCov) et 5 (KmlVar) de l’ANR IDOL (responsable : C. Genolini, Bordeaux ; financement obtenu en 2012).
Travail fondamental d’adaptation des méthodes usuelles de classification de trajectoires à des biomarqueurs binaires ou résultant d’un comptage.
Projets de recherche
Utilisation des trajectoires typiques pour établir le diagnostic précoce de maladies
Utilisation de la méthodologie d’identification de trajectoires typiques pour évaluer la capacité d’un biomarqueur longitudinal à établir un diagnostic précoce. L’application portera sur l’analyse des variations de β-HCG suite à un premier curetage de mole hydatiforme pour le diagnostic d’échec du curetage.
Prise en compte des conséquences cliniques dans le choix des tests diagnostiques
Le choix d’un test diagnostique (ou de son seuil) repose très fréquemment sur l’analyse de sa courbe ROC. Le succès de cet outil en a fait oublier la non prise en compte de la prévalence et des conséquences cliniques des erreurs de classement (maladie non détectée ou traitement à tort). Ces paramètres impactent pourtant grandement le bénéfice espéré en population de la stratégie diagnostique.
Le travail de recherche visera à adapter de façon simple les courbes ROC pour prendre en compte ces deux facteurs. L’objectif est que cet outil aussi populaire en recherche clinique soit désormais, par des modifications simples, utilisé de façon rationnelle en termes de prise de décision médicale.
Etablissement de seuils pour la médecine personnalisée
La médecine personnalisée repose sur des biomarqueurs qui orientent le choix de la stratégie de traitement. Aujourd’hui, les seuils de décision associés à ces biomarqueurs tiennent compte du caractère répondeur ou non du patient au traitement, mais rarement de la qualité de vie. La thématique de recherche portera sur les méthodes de prise en compte de l’état de santé futur d’un patient dans le choix du seuil optimal d’un biomarqueur prédictif, et sur l’intérêt de la prise en compte de ce paramètre.
